It would be our pleasure to see you. We ask you first request an appointment with your primary provider before scheduling an appointment with us. We always encourage continuity of care and will encourage you to follow up with your regular provider, after your visit, should you choose to see us. You will be provided with a summary at the end of your visit to give to your primary provider.
It is our pleasure to do this for you. We will ask you to sign a release of information during your visit and request your records.
We are currently credentialed with most major insurance carriers. Some credentialing is still pending. We would be delighted to check your coverage prior to your appointment and notify you.
Our Medicaid slots are limited, however we are credentialed to accept Medicaid as a PCP.
